Wellness Partner/ Associate Care Manager
Do you have a heart for creativity and empathy for working with older adults? Corewood Care Management is seeking college educated individuals with a passion for engaging older adults in person-centered life enrichment activities.
Use your warmth, care, creativity and socialization skills to create joy through individualized leisure activities in each person's home and/or their community.
Our Wellness Partners are an integral part of our team and provide vital support to our clients by reducing an individuals' sense of isolation and allowing the individual to maintain their independence and to thrive.
The Wellness Partner will work with our Care Management team to provide companionship and non-medical support.
Wellness Partners do not provide hands-on direct care and enjoy flexible schedules and hours.
Fun and flexible! Position requires a self-owned car to drive between client's homes and places of service.
Job Duties:
· Provide companionship by discussing topics of interest, current events, lifetime achievements or reminiscing on memories.
· Accompanying or assisting with shopping/errands (Going to a grocery store, restaurant, café, salon/barber, movies/theater, or a pleasant drive).
· Scheduling and accompanying to appointments as needed.
· Encouraging and participating in home-based activities such as cooking, organization, looking at photo albums, playing cards, games, letter writing, gardening, calendar organization.
· Research and plan activities or outings of interest for client.
· Report any concerns about the client's well-being/health to the Care Manager.
· Assist client with virtual calls and correspondence such as FaceTime, Skype, emails, Apps etc.

· Provide a written summary of the visit to the Care Manager and family members/close contacts via Corewood Care App and submit in a timely manner.
· Attend monthly Care Management meetings and trainings.
· Have the flexibility and availability to commit to clients and provide consistent companionship.
Job Qualifications:
· Associates, bachelors, or master's degree required.
Concentration or focus on Human Sciences preferred.
· Professional or personal experience in working with older adults.
· Speak fluent English.
· Clear and current background check and driving check.
· A desire to assist older adults maintain feelings of independence and self-worth and, through visits, increase their social contacts and connections.
· Ability to listen and to relate to older adults in a warm and friendly manner.
· Possess an understanding of the needs of older adults and the aging process.
· Comfortable with administrative duties and demonstrate basic computer skills.
· Must have a current and valid driver's license, possess own vehicle, proof of current car insurance and comfortable with driving others in vehicle.
· Must be flexible with your schedule and have the ability to work a minimum of 10 to 15 hrs.
· Comfortable working with clients in Montgomery County, MD and Washington DC.
· Maintain a flexible schedule to be able to meet the needs of clients.
